*Only the original owner of the new and unused vehicle is eligible. The following must be submitted for the claim: A legible copy of the owner's signed original bill of sale or purchase contract. A copy of the physicians statement identifying the specific reasons for mobility assistance. A copy of the Mobility Assistance Claim form. A copy of the purchasers current drivers license. A copy of the current registration or title. A legible copy of the paid invoices by the original owner for the mobility assistance equipment installed by a qualified installer.
